Surgical Procedures for Melanocyte Transplant

Cellular Separation:

Well-pigmented melanocytes engender pure colonies of pigmented cells, but cells separate cell type, or premelanocyte, which can differentiate spontaneously and stably in culture; this cell type includes both unpigmented and faintly pigmented cells.

Steps for Cellular Separation:

  • The epidermal layer is incubated in 0.25% trypsin EDTA, solution, in a petri dish, for 45min-60min
  •  The dermal surface should be kept above
  • This petridish is now placed in a Co2incubator at 370c, 5% Co2, for 50 minutes
  • The epidermal sheets are gently manipulated with forceps to dissociate the epidermal cells
  • Residues of dermis are discarded
  • Patients serum about 0.5-1ml is added to neutralize trypsin
  • The cells are washed with DMEM solution, centrifuged for 5 min at 1500rpm
  • Supernant solution is discarded and the cellular pallet is mixed with patients serum (1-2ml)
  • This is filled in insulin syringes, and is ready for transplant.

Although each dermal filler has unique attributes, in general, most dermal fillers work in two ways. First, most of the fillers plump and lift the skin gently to replace collagen loss caused by the natural aging process. Secondly, most of the fillers also stimulate the body to produce its own natural collagen; some help more than others. Physicians use dermal fillers in a variety of ways to lift features of the face, correct wrinkles and to restore smoothness and volume to the skin. The effects of most fillers are nearly instantaneous.  Most of the dermal fillers last from several months to a year or two before touch up treatments are necessary to extend results.

What is Acne Scar?

Acne is a common human skin disease, identified by areas of scaly red skin, blackheads, whiteheads, papules, pimple and scarring.


What causes acne?

Nobody is completely sure what causes acne. Experts believe the primary cause is a rise in androgen levels. Androgen levels rise when a human becomes a juvenile. Types of Acne Scars:

Ice pick

Ice pick scars are deep pits that are usually less than 2mm across. They extend into the skin, giving the skin an appearance of having been punctured by an ice pick.



Boxcar

Boxcar scars are angular scars with sharp vertical edges, and resemble the scars left by chicken pox. They may be shallow or deep, and are most often found on the cheeks and temples.



Rolling 

Rolling scars are caused by damage under the surface of the skin. They give the skin a wave-like appearance. They tend to be wide and shallow.



Hypertropic 

Hypertrophic scars are raised and lumpy. They tend to appear on the back and chest, but can also appear on the neck and face. Often the result of severe acne (cysts or nodules), they generally stay within the boundary of the original wound, and may decrease in size over time.



Hyperpigmentation 

Hyperpigmentation refers to the red or dark marks that are left behind after a pimple heals. These marks, also called macules, are not scars, but are often confused with scars because they can last for months or even years before they fade. Tips for Acne Preventing:

  • Wash skin carefully twice per day, and after heavy sweating.
  • Wash your face with a gentle cleanser, from the hairline to below the jawline. Rinse thoroughly and avoid harsh scrubbing.
  • Wash your hair daily, and more frequently if your hair is oily.
  • Never pick or squeeze acne blemishes, which can lead to scarring.
  • Take care to prevent sunburn. Some acne medications can make your skin more vulnerable to sunburn.
  • Look for more acne-friendly skin and hair care products.
  • Avoid sports equipment, backpacks and other items that rub against skin.

    FUE - Follicular Unit Extraction

    FUE - Follicular Unit Extraction

    This method of donor harvesting, removing follicular units one-by-one directly from the scalp, is what differentiates the FUE procedure from a traditional Follicular Unit Transplant, where the donor hair is removed from the scalp in one thin, long strip and then subsequently dissected into individual follicular units.

    Advantages:

    • No linear scar in donor area
    • Important for those who wear their hair very short
    • Decreased healing time in the donor area
    • No limitations on strenuous exercise after the procedure
    • Less post-op discomfort in the donor area
    • Useful for those with a greater risk of donor scarring
    • Useful for repairing donor scars that cannot be excised
    • Provides an alternative when the scalp is too tight for a strip excision
    • Enables one to harvest finer hair from the nape of the neck
    • Makes it theoretically possible to harvest non-scalp hair.

    Disadvantages:

    • Follicular units in FUE are harvested from a much greater area of the donor zone compared to FUT
    • Graft quality is not as good compared to FUT 
    • The maximum follicular unit graft yield is lower than with FUT
    • With each subsequent session, the scarring in FUE is additive
    • In large hair transplant sessions, the entire donor area must be shaved
    • Microscopic dissection may be needed in addition to the extraction
    • After large numbers of grafts are harvested, fine stippled scars may become visible due to thinning of donor area
    • Long-term, if the donor area narrows, the scarring may become visible
    • With FUE, grafts are usually out of the body for a longer period of time compared to FUT
